DBS DOCUMENT LIST
We are required to conduct a DBS check for all our Volunteer Team and Trustees.
We are required to check and verify your identity by checking;
ONE document from GROUP 1 below
&
TWO further documents from GROUP 1 and 2 below
We will make contact with you with the correspondence details to send the information to.
At least one of the documents must show your current address.
All data provided as part of this process will be destroyed once the DBS check has been carried out.
GROUP 1
- Passport (any nationality)
- Biometric resident permit (UK)
- Driving Licence - photocard (UK, Isle of Man, Channel Islands and EEA)
- Adoption Certificate
- Birth Certificate - issued within 12 months of applicant's birth (UK & Channel Islands)
GROUP 2
Documents from within previous 12 months
- Council Tax Statement (UK and Channel Islands)
- Financial Statement eg. pension, endowment, ISA (UK)
- Mortgage Statement (UK or EEA)
- P45 certificate statement (UK and Channel Islands)
- P60 certificate statement (UK and Channel Islands)
Documents from within the last 3 months
- Bank or building society account opening confirmation letter (UK)
- Bank or building society statement (UK, Channel Islands or EEA)
- Bank or building society statement (countries outside of the EEA, branch must be in the country where the applicant lives and works)
- Electricity bill/statement (UK)
- Gas bill/statement (UK)
- Credit card statement (UK)
- Telephone bill/statement (not mobile phone) (UK)
- Water bill/statement
- Benefit statement eg. child benefit, pension (UK)
- Document from Central or Local Government, Government Agency of Local Council document giving entitlement
Documents from the following
- Driving Licence - photocard (issued outside UK and EEA) or paper version issued without a photocard) (if issued before 1998) (UK, Isle of Man, Channel Islands, EEA)
- Firearms Licence (UK, Channel Islands and Isle of Man)
- HM Forces ID card (UK)
- Birth Certificate - issued AFTER 12 months of the applicant's birth (UK and Channel Islands)
- Marriage/civil partnership certificate (UK and Channel Islands)
- Immigration document, visa or work permit (Issued by a country outside the EEA)
- Correpondence/ID cards - Card carrying the PASS accreditation logo (UK, Isle of Man and Channel Islands)
- EEA National ID card
- Letter from Head Teacher or College Principal of a UK institution (only to be used by 16 to 19 year olds in full-time education if other documents cannot be provided)
- Letter of sponsorship from future employment provider (non-UK/non-EEA only - valid only for applicants residing outside of the UK at time of application)
- Irish Passport Card
